It depends on the NAT. If the NAT is a symmetric NAT you need a media 
relay. Other NAT types usually can be traversed by means of STUN. Thus, 
if your clients support STUN setup your own STUN server and configure 
the clients to use your STUN server (For testing you can also use any 
public stun server, e.g. sipgate.net)
